Western Dental tooth was pulled.They claimed another tooth that was already pulled by them at an earlier date. . Duplicate claim. Antioch California

I went to Western Dental on 12/24/14 for a cleaning & an upper right tooth extraction. I was convientanly charged almost 1500.00 dollars which happens to be my yearly maximum.  I ended paying 774.00 because my insurance co. claimed that there was a duplicate claim. Western Dental claimed that they pulled 2 teeth on this date. A tooth that was pulled about a year earlier and already paid by my insurance co. Therefore I got stuck paying for it. I just received received my explanation of benefits statement almost a year later.
