AmMed Direct 401K-They owe me money! Antioch Tennessee

I am a previous employee of AmMed Direct (formerly known as Diabetes Direct) and when I quit working there, I had about $300 in a 401K.  I was not allowed to withdraw my 401K when I left-I was told I had to wait 7 years to get that money.  The 7 years is up (has been up for a long time), and now I can't find the company to withdraw my 401K money that has been sitting in an account for over 7 years!  AmMed Direct's website address they provide on the internet goes to a site for a company called Arriva Medical.  I just want my money from this company that seems to have dropped off the planet!
