Bought 2 pair of Dr Scholl's Custom Fit Orthotics. One on 11/15/14. The other on 12/9/14.

Mailed on 1/20/15: the hand written form, 2 original UPCs from the packages and the original cash register receipts. Purchase and price circled.

It's MAY 1 2015 and NO rebates and NO info on the website. Very disgusted and will NOT purchase Dr Scholl's products ever again.
