DAC services is now USIS. I tried to get a copy of my DAC from USIS and to my understanding you have to be a member of USIS therefore to become a member, you must have a USDOT DAC #, Be an owner of/or qualified representative of the company and have a dot number.

What happened to the fair credit reporting act? Is it not a legal right of the driver AS WELL AS the employer to see what we are being haunted by? This sound unfair to me.

DAC report can be requested by cdl drivers

AUTHOR: Dave - (U.S.A.)

POSTED: Wednesday, March 04, 2009

driver,
yes you can request a DAC report from USIS at their website www.usis.com
then click on the link at the bottom "Consumers" and about half way down on this page is a link to the pdf version of the DAC application. you are allowed one free report ever 12 months. just check the box on the form "no charge" if you have not yet requested one. If you have in the recent year it is 15.00 per the form.

you can also order it through www.DOTjobhistory.com after signup and payment for them to get it. (this website has no affiliation with USIS.)
I must let you know though the report takes 15 days to get and through this website above will be in short form and not the full length one you could get from USIS.

hope that helps.
