We keep receiving fake invoices for an individual in UT; waste of our fax machine resources and no contact info for this company, so we can't request faxes to stop.

I wouldn't spend too much time on this. After a client and I received this invoices, we look into this company. None of the information is accurate. The address doesn't exist and the phone numbers are invalid. Unless you have call waiting on your fax machine, it is unlikely that you will be able track this company down. The good news is that without no valid address, they won't receive any money. It's probably more of a joke than a scam.

Fax It To Your State Attorney General's Office

You should fax this document to your state Attorney General's office and ask them to investigate. I think one report on this site indicates that the zip code they are using is for Ohio. Double check that information and then fax it to the Attorney General's office of that state as well.
