A friend of mine recently had a severe stroke. He has had Mega Life health insurance since 1990 through NASE, the National Association of the Self Employed. After 3 days in the local hospital that treated him immediately upon suffering the stroke, he was transferred to an acute care hospital because the services he needed were not available at the general hospital. He was unable to speak and partially paralyzed with medical issues that required 24 hour monitoring and treatment.
Astounding as it might seem, Mega Life has informed us that they do not cover any treatment or rehabilitation after the initial three days in the local hospital. This is the response to a customer of almost 20 years that purchased the top level of medical coverage available.
There are probably tens of thousands of others with such coverage through NASE or other organizations. Is a class action suit necessary to protect everyone?

There have been many suits filed against this company, including state insurance regulator complaints and so on. I used to work for this organization many years ago and all I can tell you is, their coverage is filled with holes and outs for them to deny claims.
IMHO, I would contact the attorney general in your state and file a formal complaint.
