I finally managed to get through on their toll long distance number and spoke to a person who describes himself as Elliot.
He told me he would send out another one within four to five days and that was three weeks ago. Still have not received anything. Tried to call back with a busy signal all the time.
Emailed customer service and of course just like before there is no response.
