Received invoice from UST dated 6/15/12.  Description is for Telecom Maintenance Agreement in the amount of $425.00.  Remit to address is PO Box 970 in La Verne CA.  Phone number 800-217-9437 is operative but no one ever answers.  Listen to the music forever or press 1 to leave a message.  

The Office of the Attorney General in Bismarck, North Dakota has filed a Cease and Desist Order, notice of civil penalty and notice of right to request hearing to David Bell CEO of US-Telcom.  

Other noted addresses for David Bell are 1967 S. Myrtle Ave Monrovia, CA 91761-7619,  305 N. Sacramento Ave in Ontario California 91764-4423 and 2001 E. Elm Street also in Ontario California.

Terms and Conditions are completely lorem ipsum and should not be taken seriously.
