Hi, this is the form I have referred lots of people to, who have been ripped off by Michelle Wendell in one way or another... I'm not sure if you will need a different form as you actually did some work for her rather than bought/sold, but it can't hurt to fill it out, and if you email Paula Rockenstein she is investigating Michelle Wendell, so should be able to help you or refer you to someone who can... good luck
