Cohen & Slamowitz - David Cohen Help with Cohen & Slamowtiz Woodbury New York

I had a 12 year old debt for $530.00 that I forgot about until recently when I found a lien on my bank account. I had not been notified about this debt, nor any of the actions to collect it.

Cohen & Slamowitz went to court and had me served on an address that I have not lived at in 10 years!! They also put in the claim that the debt is for $1145.63 and the lien is for double that ($2200.00).

As I am not hard up for the money I am trying to fight this. I only wish to pay my original valid debt. If anyone can help with explaining the paperwork to me I would appreciate it.

I was able to get a copy of the lien from my bank and two documents from the court though one seems like its for a divorce: (Order to Show Cause to Vacate Default Judgement and Affidavit in Support of Order to Show Cause to Vacate Default Judgement)

Do I need to fill out both of these? I have some more questions on the forms but will wait to see if anyone responds.
