United Healthcare abused and harassed me. I have medical conditions in my lumbar sacral spine and cervical spine both with radiculopathy. I have all my medical treatments, mris, diagnosis, medical doctors and phd. reports. These show that I cannot pemanently, get employment doing prolonged standing, walking,lifting or carrying. I am on disability about four years. Mostly been on United Healthcare medicaid. I would walk into a provider, they see the type of insurance you are on and treated badly.Info I read said that health insurance companies can call any of your providers and say negative bias comments about you particularly to try and get you off your disability. I have had my mris,doctors reports, treatments,etc., ignored and obliterated from clinics I go to. I have them faxed back in, proofs they go through, and then ignored by providers or not in clinic system again. I have all my records however.

The straw that broke the camels back was after I read a horrible physical therapy report, when I was treated for plantar fascitis in June July 2012. I read this on about October13,2014. United Healthcare payed them for this. The plantar fascitis was changed to,ankle joint pain right ankle,with feet pain,which in my only opinion,was done to make it look like I did not have a bad back or irritable l5s1,and radiculopathy.I wrote in forms other med hx. but the PT put in her initial evaluation nothing,not even the plantar fascitis that was in the diagnosis referal,and she put I changed topics and she so at evaluation she had no past or present info to put down. The exercise was switched to standing on steps,raising heel up and standing on toes.several times each foot. PT treated me for plantar fascitis, heels down, lots of those dorsiflexion exercises,I never did the heel raises in the sessions or at home like she said,did the plantar fascitis ones in sessions and home,that she gave me printouts. A PTA did me once and I did my exercises before he came in, so I told him i did he checked what he saw on list. PTA had again and not checked as I did not do. Last day PT had me do several heels up each foot on steps and i got low spine ache and never did again. PT made like my right ankle and I could not do plantar fascitis exercise,but why stop plantar fascitis treatment on left? And why treat left for ankle joint pain? Never had ankle joint pain. Anyhow, I made provider complaint oct.23,2014 and oct27,2014 over phone to United Healthcare about the physical therapist,contents of changed record.

I faxed UHC about inciden,last fax,november2,2014.Nov.3,2014 and Nov.4,2014 UHC service coordinator calls me to call her about nonexistant report I never made. I saw bad scene,theybias,and blew it off. I wanted nothing to do with UHC anymore.Next in mail I get five provider reports i never made sent to me by UHC. All the same.They had the company name of PT place. and one time PT name.All saying same thing, we received your provider complaint,then next letter said,we aready reviewed this complaint and saent you letter explaining decision,since duplicate,cannot process as appeal.Then the UHC put in my patient I.D.files!.Five of these dated,Nov4.Nov7,Nov.17,Nov.25 and Dec.4,all in 201. My only opinion is that they were trying to protect PT and themselves by making me look like a nut job.

Dec.5 and 8,2014,got very phony pranky sounding answer machine message from UHC service coordinator to call her about my requests for therapy! Gee, what kind? Sounds like an innuendo.I had wriiten faxes in nov.2014,to UHC that I not make any provider complaints and to send me files,and they ignored. Never even got initial PT case resolution.that they never acknowledged in first place,and of course not interested in anything to do with UHC.Had no phone,verbal or written contact,except faxed,last one nov.2014,to take fake reports out. Saw one file of the five fake reports i never made. Just a photo copy of original exact report.I contacted medicaid ombudsman and told them I needed the files,and I am done withUHC,not asking them for them If I could pick files up or mailed to me,and told ombudsman I needed fake cases taken out of my file.

Well,just got letter sent to DHS my complaint,and complaint changed.It says,UHC they told to call me about my benefits,and for me to call them about!I sent ombudsman a few faxes, no phone calls. What an obnoxious repulsive situation all this is. BY the way, Months ago i dropped UHC,and have other insurance,but still on medicaid.

I am author of above, anonymous or G.M. Received dated letter January,21,2015 from Texas Department Of Human Services Commission, 4900 North Lamar, Austin,Texas. Said Health Plan Managemrnt is writing to let me know they received my complaint regarding benefits with United Healthcare. What complaint? And it goes without saying I have nothing to do with United Healthcare anymore. UHC not even my insurance anymore. Officially dropped them in December2014 and received new dated other health insurance medicaid card January1,2015. Initially I assumed that when I faxed the ombudsman in December2014, requesting all in writing I will add, that they sent it to DHS. The request for the five bogus provider complaint appeal requests files, that I never made to UHC that the UHC put in my patient I.D. medicaid files. Taking the focus off of the initial real complaint on scam P.T. JuneJuly2012 that was never acknowleged and will not be.I have heard nothing since either way. The DHS bogus complaint makes the sixth impersonated hoaxed report I never made. I would never have wanted to or would want to contact DHS or medicaid regarding issue. UHC con artists called me saying they would call three times to try and contact me about fake complaint whatever it was from DHS,and to call them about,right after above letter they called. Biggest scam cons. Of course I never called back. Also, in past a third party trying to get the five impersonated hoaxed appeal request provider complaints files I never made obtained to give to me informed me that United Healthcare told them to tell me to contact medicaid! United Healthcare treats you with futile absurd gaslighting, and treats you like you are braindead!!!
