After cancelling service with this company because of dissatisfaction, I have been waiting for a credit to my credit card as I overpaid my bill.  I have received 2 statements showing a cfredit balance.  I returned the set-top boxes and have sent the company emails showing the tracking numbers, date of receipt, and who signed for the return.  I have been waiting for over 30 days for the refund and finally was able to reach the office of the president.  Please note that customer service will not provide his name or phone number.  In fact, they give you an incofrrect mailing address to reach him  His name is Michael White.  His phone number is 310-535-5000 and correct mailing address is 2230 E Imperial Hwy, El segundo, CA 90245 (Not the Colorado address on the statements!)
Here is the last response I received from this company:

We are writing in regard to recent correspondence we received from you.  On behalf of DIRECTV, we want to thank you for taking the time to write to us.  We appreciate the opportunity to respond to your concerns.


After reviewing your account, our records show you contacted our office on 01/25/10, and we noted the equipment Non-Return Fee was submitted for credit to apply within seven days.  Once credit is applied, a refund will process to the credit card on file within 30 days.  Our research confirms your issue was addressed by DIRECTV and an additional response is not necessary.

Obviously, they wish to keep your money as long as possible so they can earn interest and cause you to run up interest on your credit card.  These people are total incompetents and do not deserve to remain in business!
