La Boxing Doctored my contract and changed it for a 1yr contract from a 90 day contract Guaynabo, Other

I signed up for a gym membership at LA Boxing in Guaynabo in July of 2010 and wanted a 90 day. I went to one training session and during that training session one of the instructors by the name of Omar who is a profession MMA fighter decided to take it upon himself to use me as a sparring partner for him his cousin and another gym. I informed Omar that I was not there to fight because I already did my fighting in iraq and i just wanted to work out. After giving me a fat lip and messing my back up, I took a break from LA boxing. not to much later I had to return to my home in New York and needed to cancel my membership.
I called my ASF and let them know what was going on and that I needed to cancel my contract. I spoke to a man named mike and he said that as long as I sent in the paperwork stating that i was receiving mail at a new address they would cancel my membership and waive my cancellation fee. I was in the process of checking my and saw that my membership was not canceled. I called MY ASF and they informed me that my paperwork was not good enough and that they needed something in my name in order to cancel my membership!
I provided them with ample documentation and infact told them that my contract was supposed to be on a month to month basis now that my initial contract was done! They would not cancel my membership and they are now starting to become annoying in the fact that they are not allowing me to cancel my membership even with all the proper documentation

My ASF and LA Boxing are complete rip offs and need to be shut down!
