Alamo Charged me again for CDWs etc. Sanford Orlando, Florida

I rented a car at Sanford Airport (Orlando).  I had prepaid for the car using a package deal with Cosmos, which clearly states that CDWs and unlimited mileage etc. are included.  The guy asked me if I wanted to add another driver, GPS, re-fuel on return, baby chair and I said no.  Then he said to initial the forms - I did not know that I was authorising another $405.40 to be added to my bill for Collision Damage Waiver and Extension - Time and Distance!  In the past, I have always been asked if I want that and I always refuse.  I complained to Alamo online and they said tough luck and that I should have read what I was agreeing to!
I say that this is fraudulent because they did not ask me if I wanted it and effectively charged me twice over for the same thing.
