Pure Fitness Fraudulant overcharges, failure to cancel contract, violation of FDCA Tempe Arizona

I joined the gym and signed up for EFT through my credit card. I canceled my membership within 90 days as I was moving out of state. I went into the gym and was given the cancellation forms, which I completed and handed back to the manager. I was told my card would not be charged any further and the membership would be canceled.

My card was charged for almost 1 year after the cancellation. I called the gym numerous times and was told not to worry. The gym would refund the overcharges.

Pure Fitness turned the account over to a collection agency, Vision Financial, and the collection agency has continued to harrass me to collect on an account that was canceled and owes me for fraudulant charges to my credit card.

I cannot get this resolved.
