 In March of 2010, I had surgery at St. Elizabeth hospital to repair a torn tendon. The surgery went well, but my recovery did not. My incision did not heal, and were it not for my surgeon keeping on top of it, I would have lost my foot. He sent me to your wound clinic, per clinic policy. In the wound clinic I saw an actual doctor once. He looked at my wound, told the nurse to clean it, put some antibiotic ointment on it, and wrap it up. I had to come in twice a week for four weeks, for the same treatments. The wound continued to get worse, and nobody from the wound clinic took responsibility for my care. My surgeon finally said enough is enough and had me come in for a wound debridement. Because it was so bad, I had to be put under, which was yet another surgical procedure. Due to the negligence of those employed in your wound clinic, I ended up with a permanent disability. In august of 2010, the wound had finally healed to the point where I could return to work part time. I had been diagnosed with RSD, and I'm only able to wear flip flop type shoes and have nothing touching my ankle and foot. My part time employment ended in March of 2011. I was terminated because my employer, Time Warner Cable, could no longer accommodate my restrictions. This included my state of Wisconsin issued handicap accessible parking permit. They had sent an email stating the handicap parking stalls closest to the employee entrance were reserved for those using wheelchairs. This meant I had to park a distance away from the entrance and walk through the unplowed parking lot and sidewalk in flip flops. When I was terminated, I was told by the HR rep that my active employee benefits would continue through august of 2011. I knew I had a surgical procedure scheduled in April of 2011 so I verified with the HR rep that my insurance would for sure remain active. She assured me that it would. Had I had ANY indication that I wasn't going to be covered by my insurance, I could have easily gone under my husband’s plan. I was also told that after August of 2011 I would have the option of going under COBRA insurance. I was never sent any information on how to partake in this benefit. My final paycheck from Time Warner Cable reflected my portion of the health insurance premiums that would have covered me at least through June of 2011. According to Time Warner Cable, there was a "clerical error" which resulted in my health care benefits being terminated as of March 31, 2011. I have to question why it took until May of 2013 for TWC to come to this conclusion. They had the money that was deducted from my payroll for the employee portion of my health insurance premium. Why wasn't this paid to the health insurance provider? Where was this money? Did it not show up in audits? I had made several phone calls to corporate as well as the local HR department to try and get answers to this issue when I was first made aware in approximately July of 2011. None of the people I spoke to were willing to help me. Because TWC employees failed to follow procedure, file paperwork in a timely fashion, notify corporate benefits of status changes, and follow through with exit interview policies my health insurance coverage was prematurely terminated. This was an error on behalf of Time Warner Cable, and in no way reflects my failure to comply. My husband and I are now at risk of losing everything we have worked for. We are facing financial ruin and this is causing extreme stress and hardship on our marriage. Due to the negligence of Time Warner Cable we had to file for bankruptcy. We are now trying to rebuild our credit, and we are diligent about paying our bills on time. Because Time warner cable is the only cable and Internet service provider in my area, I had to stay with them for my services. They refused to provide me with a bill detailing my monthly services and charges. I have been trying to get this rectified for over 5 years, and they keep telling me it is resolved. And actually, it was working for a while, but then it started up again. I would get a bill for $0.00 and no monthly charges. As soon as this happened, I contacted them to get it resolved. I have been hung up on, more than once, told it would be looked into, told it was resolved, yet it still is not taken care of. I will not just give my money to a company because someone says this is what you owe us. I believe I have a right to view a billing statement so I can verify the services and charges associated. They have refused to send me a bill, they have refused to send me a copy of my account informTion, including the customer comments and /or notes. I believe by law, they have to provide this information upon request. I do not deny that I have been getting service, most of the time. I could have easily said nothing, and continue to get services free of charge. I have made every effort to be honest and get this resolved. Time Warner Cable has refused to comply and has forced me to do the work on their behalf. After several calls on my behalf, they simply transferred six months of charges to another account in my name and disconnected me for non payment. They sent me a copy of that statement with no explanation, or assurance that the issue was resolved. Now, once again, because Time Warner Canle refuses to take responsibility for their errors, I have an account with a collection agency.

even though I have not had service from Time Warner Cable in over 4 months I continue to get monthly email alerts saying I have a bill due soon. When I try to view this I obviously cannot log in as I have no active account.
